On a daily basis, home care service providers reach frail older adults who reside independently in the community. Home care clients are at very high risk of injurious falls, which typically are the sentinel events for relocation to long-term residential care.

Mobile at Home initiatives restore mobility (increasing home management and self-care capabilities), reduce risk of falls and prevent further mobility decline among older adults in receipt of home care services. ABC achieves this through provision of falls and falls-related injury risk prevention training and ‘re-ablement’ activities.
